Contract Position Pay:

$60-$90 per visit, based on visit type and experience Attentive Home Care Services is seeking a dependable and compassionate Florida-licensed Physical Therapist (PT) to join our growing home health team. This is an excellent opportunity for a PT looking for flexible scheduling, independence, and meaningful one-on-one patient care in the home setting. What You'll Do Complete initial PT evaluations and reassessments Develop individualized therapy plans and measurable goals Provide skilled physical therapy in patients' homes Perform gait, balance, strengthening, mobility and transfer training Assess fall risk and home safety Educate patients, families and caregivers Monitor patient progress and modify treatment plans as appropriate Coordinate care with physicians, nurses, PTAs and other members of the interdisciplinary team Provide appropriate supervision of PTAs when applicable Complete accurate and timely clinical documentation Follow Medicare, payer, agency and professional standards Qualifications Current, unrestricted Florida Physical Therapist license Graduate of an accredited Physical Therapy program Current CPR/BLS certification Valid driver's license Reliable transportation and current automobile insurance Ability to successfully complete required background screening Strong assessment, communication and documentation skills Ability to work independently in the home setting Home health experience preferred Why Join Attentive Home Care Services?
