Duties Conduct comprehensive evaluations of patients' physical functions, including mobility, strength, flexibility, and pain levels. Develop and implement individualized care plans aligned with clinical best practices and patient goals. Administer a variety of physiotherapy treatments such as manual therapy, trigger point therapy, therapeutic exercises, and modalities like ultrasound or electrical stimulation. Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless continuity of care across inpatient, outpatient, home health, and community settings. Maintain accurate and detailed clinical documentation adhering to healthcare standards and electronic health records (EHR) management protocols. Educate patients and their families on injury prevention, self-care techniques, and rehabilitation exercises to promote long-term wellness. Assist in outpatient clinic operations by managing patient schedules, documenting progress notes, and ensuring compliance with clinical documentation standards. Qualifications Valid license or certification to practice as a Physical Therapist in the relevant jurisdiction. Proven experience practicing in a physical therapy role within acute care, inpatient hospital settings, outpatient clinics, or home & community care environments. Strong knowledge of physiology, anatomy, orthopedics, neurology, pediatrics, geriatrics, and related medical terminology. Demonstrated ability to develop effective care plans tailored to individual patient needs across diverse populations including geriatric patients and pediatric clients. Experience working with electronic health records (EHR) systems or electronic medical records (EMR) systems for documentation and treatment tracking. Familiarity with inpatient experience, hospital procedures, outpatient rehabilitation center protocols, and home health services. Excellent communication skills for patient education and interdisciplinary collaboration. Ability to perform discharge planning efficiently while adhering to clinical documentation standards.
