Job Summary:

Uplift Therapy Group is seeking a dedicated and compassionate Home Health Physical Therapist to join our growing South Florida team. We specialize in providing one-on-one, patient-centered care that empowers individuals to regain mobility, independence, and confidence in the comfort of their own homes. Our ideal candidate is professional, reliable, and passionate about improving lives through quality therapy and personalized attention.

Responsibilities:

Perform comprehensive patient evaluations and create individualized treatment plans. Deliver skilled physical therapy interventions to improve strength, balance, and mobility. Educate patients and caregivers on home exercise programs and safety techniques. Collaborate with nurses, physicians, and other healthcare professionals to ensure coordinated care. Maintain accurate and timely clinical documentation (per home health and agency standards). Monitor progress, adjust plans of care as needed, and ensure patient goals are achieved.

Qualifications:

Doctor of Physical Therapy (DPT) or Master's in Physical Therapy. Active Florida PT License (required). Prior home health experience preferred , but not mandatory. Strong communication, clinical reasoning, and documentation skills. Reliable transportation and valid driver's license.
