Job Description

Tender Touch Rehab Services seeks a full-time Physical Therapist Floater to provide compassionate, high-quality care across multiple skilled nursing and long-term care facilities. In this traveling onsite role, you will evaluate residents, create individualized treatment plans, and deliver evidence-based interventions to improve mobility, safety, and independence for geriatric and sub-acute populations. You'll collaborate closely with interdisciplinary teams, document care accurately, and support coverage needs across partner sites while benefiting from strong clinical support, professional growth, and leadership development opportunities in a collaborative, patient-focused culture. Responsibilities Provide physical therapy evaluations and develop individualized treatment plans for geriatric and sub-acute residents across multiple partner facilities Deliver evidence-based therapeutic interventions to improve mobility, strength, balance, and functional independence Float between assigned skilled nursing and long-term care sites to support census needs and provide clinical coverage Collaborate closely with interdisciplinary teams, including nursing, occupational therapy, and speech therapy, to coordinate resident-centered care Document assessments, progress notes, and discharge summaries in accordance with company, facility, and regulatory standards Educate residents, families, and facility staff on home exercise programs, safe mobility, and fall-prevention strategies Participate in care conferences, quality initiatives, and clinical programs that support high-quality outcomes Maintain professional licensure, stay current with best practices, and engage in ongoing clinical education Contribute to a collaborative, compassionate work environment aligned with Tender Touch Rehab Services' values Support mentoring or orientation of new therapists or assistants as assigned Required Skills Physical therapy evaluation and treatment planning Geriatric and sub-acute rehabilitation techniques Therapeutic exercise and neuromuscular re-education Balance, gait, and fall-prevention interventions Functional mobility and ADL training Electronic medical record (EMR) documentation Interdisciplinary care coordination Patient and family education Regulatory and compliance documentation in SNF/LTC settings Time management and schedule flexibility across multiple sites
