About Us Emergent Therapy Center LLC is a growing home health agency serving patients throughout Polk County. We are committed to providing compassionate, high-quality care in the comfort of our patients' homes. We are currently seeking a dependable and motivated PRN Physical Therapist to join our team as we continue to expand our services. Position Summary The Physical Therapist will evaluate patients, develop individualized treatment plans, provide skilled physical therapy services, supervise Physical Therapist Assistants (PTAs), and collaborate with physicians and the interdisciplinary team to achieve the best patient outcomes. Responsibilities Perform initial physical therapy evaluations and reassessments. Develop and update individualized plans of care. Provide skilled home health physical therapy services. Supervise and direct PTAs in accordance with Medicare and state regulations. Complete documentation accurately and on time using our EMR system. Communicate with physicians, nurses, and other healthcare professionals regarding patient progress. Educate patients and caregivers on exercises, safety, and fall prevention. Qualifications Current Florida Physical Therapist license. Graduate of an accredited Physical Therapy program. Current CPR/BLS certification. Valid driver's license, reliable transportation, and auto insurance. Home health experience preferred but not required. Knowledge of Medicare documentation is a plus. Excellent communication and organizational skills. TB screening Hepatiti B form What We Offer Flexible PRN schedule. $90-$130 per evaluation and $65-$90 per routine visit. Supportive leadership team.
