Deliver compassionate, patient-focused care as a Home Health Physical Therapist Assistant (PTA), providing therapeutic interventions right in patients homes as part of a collaborative healthcare team. Youll play a vital role by assisting in the implementation of individualized physical therapy treatment plans under the supervision of a licensed Physical Therapist, supporting your patients mobility and independence.

Schedule:

Monday through Friday, 8:00 AM 5:00 PM 13-week contract Approximately 2532 points per week (typically 57 patient visits per day) Daily travel within your assigned territory (mileage reimbursement provided)

Key Responsibilities:

Deliver skilled physical therapy care according to plans developed by the supervising Physical Therapist Guide patients through therapeutic exercises, gait and balance training, and functional mobility tasks Track treatment responses, document progress, and keep accurate records using Home Care Home Base (HCHB) Communicate patient progress and condition changes to the supervising PT and team members Educate patients and caregivers on safety, fall prevention, and home exercise routines Attend team meetings and case conferences as needed Follow agency procedures, regulatory standards, and all home health compliance guidelines

Preferred Qualifications:

Active PTA license in good standing for the state of practice Minimum 1 year of recent home health PTA experience (field-based strongly preferred) Working knowledge of HCHB and OASIS documentation processes preferred Excellent organizational, time management, and clinical observation skills Ability to work independently in a field-based setting Reliable transportation, valid drivers license, and auto insurance
