Duties Assess pediatric patients' physical conditions by evaluating their physiology, anatomy, and functional abilities to inform tailored therapy interventions. Design and execute comprehensive care plans focused on improving mobility, strength, and coordination for children with various conditions. Collaborate with multidisciplinary teams to coordinate discharge planning, ensuring seamless transitions from inpatient to outpatient or home & community care settings. Educate families and caregivers on therapeutic exercises, home modifications, and community resources to support ongoing patient progress. Maintain accurate documentation within electronic medical record (EMR) systems, including progress notes, treatment plans, and discharge summaries. Monitor patient progress regularly, adjusting care strategies based on clinical observations and evolving needs in inpatient, outpatient, or home health contexts. Skills Strong knowledge of physiology and anatomy relevant to pediatric development and rehabilitation. Expertise in developing detailed care plans that incorporate discharge planning and patient-specific goals. Familiarity with medical terminology and documentation standards within EMR systems. Excellent communication skills for educating families and collaborating with healthcare teams.

Pay:

$75,000.00 - $85,000.00 per year
