Full Job Description Role Overview:

This position involves providing primary physical therapy care in patients' homes within a designated five-county area, including rural locations. The therapist supervises PTAs and Home Health Aides, develops and adjusts care plans, and collaborates with multidisciplinary teams to ensure positive patient outcomes.

Key Responsibilities:

Assess, coordinate, and manage patient needs according to physician orders, from admission through discharge. Perform routine and complex therapy procedures tailored to individual patient requirements. Supervise and review the care provided by PTAs and Home Health Aides, ensuring adherence to care plans. Educate patients and caregivers to promote self-care and independence. Communicate patient progress and changes with physicians and team members. Ensure compliance with regulatory guidelines, complete documentation accurately and timely, and participate in quality improvement activities. Contribute to staff education, participate in performance activities, and support ongoing professional development.

Qualifications & Skills:

Graduate of an accredited physical therapy program with licensure as a Physical Therapist. Minimum of 1 year of full-time physical therapy experience, preferably in home health. Current BLS certification, valid Georgia driver's license, and private vehicle insurance. Strong assessment, rehabilitation, communication, organizational, and problem-solving skills. Proficiency with computer systems, home health software, and understanding of regulatory processes; knowledge of OASIS is preferred.
