Full Job Description Role Overview:

This position involves evaluating and treating patients primarily in Rehab and LTAC hospitals based on business needs. Responsibilities include conducting initial and discharge evaluations, developing and implementing treatment plans, educating patients and families, documenting progress in the EMR, and communicating with team members, physicians, and vendors. The role also includes preparing equipment orders, serving as a mentor for interns, and participating in skills checks.

Qualifications & Skills:

Minimum Bachelor's Degree in Physical Therapy; Doctoral Degree preferred At least 1 year of experience in Rehab, SNF, LTACH, or Acute settings GA PT license or eligibility and BLS certification Basic computer literacy

Additional Duties:

Contribute as part of an interdisciplinary team, serve as a Clinical Instructor/mentor, and assist in hospital-specific skills assessments.
