Role:
We are seeking a compassionate and skilled Physical Therapist to join our team at Brown's Health and Rehabilitation Center. As a Physical Therapist, you will be responsible for evaluating residents, developing individualized treatment plans, and providing therapeutic interventions to help residents improve or restore mobility, reduce pain, and achieve their highest level of physical function.
Key Responsibilities:
Conduct comprehensive physical therapy evaluations and develop personalized care plans based on clinical findings and resident goals. Deliver physical therapy treatments as prescribed, including therapeutic exercises, gait training, balance activities, and functional mobility. Monitor and document residents' progress, response to treatment, and adjust plans as necessary. Collaborate with an interdisciplinary team including nursing, occupational therapy, speech therapy, and physicians to provide holistic care. Educate residents, families, and caregivers on therapy programs, techniques, and safety measures to support functional independence. Comply with all regulatory, licensing, and facility policies including documentation and infection control standards.
Qualifications:
Graduate of an accredited Physical Therapy program. Current and valid Physical Therapist license. CPR certification required (or must obtain upon hire). Prior experience in a skilled nursing or long-term care setting preferred. Strong communication, clinical, and organizational skills.
