Department Specific Duties Mercy Home Care:
Manages a case load of patients in a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services and equipment required to meet the patient's health and human service needs Scheduling patient visits per regulation. Demonstrates ability to function independently. Demonstrates tolerance of home environment and diverse lifestyle of patient/family Knowledge, Skills and Abilities In addition to the technical and professional skills required to provide excellent physical therapy to patients, the clinician must have strong interpersonal skills, ability to motivate, good written and verbal communication skills. Professional Experience One year experience preferred. Education Education sufficient to receive licensure in the state of Iowa Licensure, Certification, Registration License to practice as a Physical Therapist in the state of Iowa or compact state required. Current certification in Basic Life Support (BLS), in accordance with the American Heart Association required. Iowa Dependent Adult Abuse/Iowa Child Abuse Mandatory Reporter training required. Employee must submit certification of completed training to Human Resources prior to start or transfer date. Mercy Home Care- Current and valid driver's license, own transportation and automobile insurance.
Physical Requirements Heavy:
