Make a meaningful difference in the lives of our residents Help residents improve mobility, strength, balance, and independence Develop individualized treatment plans based on each resident's needs Work collaboratively with nursing, therapy, and other healthcare professionals Support residents in achieving their functional and rehabilitation goals Enjoy the flexibility of a per diem schedule Be part of a caring and supportive rehabilitation environment Position Responsibilities Evaluate residents and develop individualized physical therapy treatment plans Provide therapeutic exercises, functional mobility training, gait training, balance activities, and other appropriate interventions Monitor and document residents' progress and response to treatment Modify treatment plans as needed based on clinical findings and resident progress Collaborate with the interdisciplinary team to support quality resident care Educate residents, families, and caregivers on appropriate exercises, mobility techniques, and safety strategies Maintain accurate and timely clinical documentation in accordance with facility and regulatory requirements Promote resident safety, independence, dignity, and quality of life Qualifications Current Physical Therapist (PT) license in the state of Iowa Graduate of an accredited Physical Therapy program Strong clinical, communication, and organizational skills Experience in skilled nursing, long-term care, rehabilitation, or geriatrics preferred Compassionate and patient-centered approach to care Ability to work independently while collaborating effectively with an interdisciplinary team Ready for a Fresh Start This Fall?
