Job Description

Physical Therapist
  • PT Home Health, Urbandale, IA
  • Local or Travel 13 weeks. M-F, 8am
  • 5pm, 40 hours per week.
On-call info:
On-call required if requested
  • on-call needs are subject to change based on staff and branch needs
Productivity Units:
30 units
Qualifications:
HCHB, Oasis, and HH experience required
Skills:
Current State License as a Physical Therapist
  • Required. Proof of current CPR certification
  • Required. Current driver's license. Must have reliable transportation and insurance
  • Required 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting
  • Required Previous home care experience
  • Preferred.
