Role:

As a Physical Therapist at Hope Creek Nursing and Rehabilitation Center, you will play a vital role in helping our residents achieve their optimal level of physical function and well-being. You will work closely with our interdisciplinary team to develop and implement comprehensive rehabilitation plans, focusing on patient-centered care and outcomes-driven treatment. Your expertise and passion for physical therapy will enable our residents to regain strength, mobility, and independence, empowering them to live their best lives.

Responsibilities:

Conduct thorough initial and ongoing assessments to identify patient needs and develop individualized rehabilitation plans Implement evidence-based physical therapy interventions, including exercise programs, manual therapy, and modalities Collaborate with our healthcare team to develop and update patient treatment plans, ensuring seamless transitions and optimal outcomes Conduct patient education and training on proper body mechanics, safety, and self-care techniques Maintain accurate and detailed records of patient progress and treatment plans Participate in quality improvement initiatives to drive continuous improvement and excellence in patient care

Requirements:

Must be a licensed Physical Therapist (PT) with a degree from an accredited Physical Therapy program Must have a strong foundation in physical therapy principles, techniques, and best practices Must be able to work effectively in a fast-paced, dynamic environment with a focus on patient-centered care Must be able to lift, push, and pull equipment and supplies as needed
