What You'll Do As a Home Health Physical Therapist, you'll: Complete in-home PT evaluations and visits Develop individualized plans of care Help patients improve mobility, strength, safety and independence Educate patients and caregivers Monitor patient progress Complete required clinical documentation Coordinate with the patient's care team as needed

What We're Looking For Required:

Current Illinois Physical Therapist license Reliable transportation Ability to travel to patient homes within your selected coverage area Strong communication and documentation skills Home health experience is preferred but not required. Experience in rehabilitation, skilled nursing, outpatient or acute care settings is also welcome.
