Job Description
Olympia Fields Hospital seeks a compassionate Physical Therapist to evaluate and treat patients across inpatient and outpatient settings. You will perform functional assessments, develop evidence-based treatment plans, deliver therapeutic exercise and manual therapy, and educate patients and families on mobility, safety, and home programs. Collaborate with a multidisciplinary team, document care in the EMR, support discharge planning, and contribute to quality, safety, and continuous learning in a community-focused environment. Responsibilities Perform comprehensive physical therapy evaluations and functional assessments Develop and implement individualized, evidence-based treatment plans Provide therapeutic exercises, manual therapy, and neuromuscular re-education Deliver gait, balance, and mobility training to improve function and safety Educate patients and families on care plans, home exercises, and fall prevention Document assessments, goals, and progress in the EMR accurately and timely Collaborate with physicians, nurses, and other clinicians on patient care Support discharge planning and safe transitions of care Participate in quality improvement and patient safety initiatives Contribute to a patient-centered, compassionate, and collaborative culture Required Skills Physical therapy evaluation Treatment planning Therapeutic exercise Manual therapy techniques Neuromuscular re-education Gait and balance training Patient and family education Electronic medical records (EMR) documentation Interdisciplinary collaboration Discharge planning
