Contract Position Compensation:

Based on Experience About the Opportunity We are partnering with an established, multidisciplinary pain and wellness clinic in the Rockford/Loves Park area to find a Physical Therapist (PT) to join their collaborative team. This opportunity is ideal for a clinician who enjoys patient-centered care and takes an integrated approach to rehabilitation, pain management, and overall wellness. The practice focuses on individualized treatment solutions designed to help patients improve mobility, restore function, and achieve sustainable health and wellness outcomes. Position Overview The Physical Therapist will evaluate patients, develop individualized treatment plans, and deliver physical therapy services to support rehabilitation, functional improvement, and patient independence. This role involves collaborating with other healthcare professionals, educating patients and caregivers, and adapting treatment programs to meet each patient's needs. Key Responsibilities Conduct comprehensive patient evaluations, including medical history, systems reviews, and appropriate tests and measurements. Develop individualized treatment plans, establish goals, and implement evidence-based therapeutic interventions. Deliver rehabilitation services in accordance with established plans of care and patient schedules. Provide patient and caregiver education to promote independence, safety, and long-term wellness. Utilize a variety of physical therapy techniques, including: Therapeutic exercise and strengthening Balance, coordination, and gait training Functional training and postural stabilization Manual therapy Flexibility, endurance, and mobility exercises Appropriate physical agent and mechanical modalities Collaborate with rehabilitation team members and other healthcare professionals to support comprehensive patient care. Make referrals to other disciplines when appropriate to address patient needs. Adapt treatment programs based on patient progress, capabilities, and clinical needs. Maintain accurate and timely clinical documentation and patient records. Participate in patient care meetings, staffing conferences, and other related discussions. Promote a safe, compassionate, and patient-centered clinical environment. Qualifications Current Physical Therapist license (preferred). CPR certification (preferred). Strong clinical assessment, communication, and patient education skills. Commitment to individualized, patient-centered care. Ability to work collaboratively within a multidisciplinary healthcare environment. Compensation & Schedule Compensation based on experience. Hourly or salaried compensation structure.
