Weekdays; weekend/on-call requirements to be discussed Make a Difference as a Home Health Physical Therapist We are seeking an experienced Physical Therapist (PT) to join a home health team serving patients throughout the Mokena and Frankfort, IL area . This is a great opportunity for a PT who enjoys providing one-on-one care in patients' homes while having the independence of working in the field. Position Highlights 13-week assignment with a 9/21/2026 start date 8-hour daytime shifts Home health visits throughout the Mokena/Frankfort territory Meaningful one-on-one patient care Supportive clinical environment Weekend and on-call expectations to be discussed Requirements 1+ year of Home Health Physical Therapy experience

Active PT license/credentials required for the assignment What You'll Do Provide physical therapy services to patients in their homes Evaluate patients and develop individualized treatment plans Help patients improve mobility, strength, balance, and independence Document patient progress and maintain accurate clinical records Collaborate with physicians, nurses, and other healthcare professionals Travel throughout the assigned Mokena/Frankfort territory Why Consider This Opportunity?
