Evaluate physical therapy needs of pediatric to geriatric patients in a home health setting Develop personalized treatment plans aimed at improving mobility and independence Implement, monitor, and adjust patient care programs effectively Collaborate with patients, families, and healthcare team members Document all care activities thoroughly in electronic medical records Assist with patient transfers and positioning as needed Communicate treatment progress and outcomes with the care team
