1 Description American Traveler is seeking a Home Health Physical Therapist with at least 1 year of home health experience to provide skilled care to patients recovering at home in the Mokena/Frankfort area. Details Home health setting covering the Mokena/Frankfort territory 8-hour day shifts Provides age-appropriate care for patients ranging from pediatric to geriatric Mileage reimbursed at the standard IRS rate Requirements Minimum 1 year of Home Health PT experience required BLS certification required IL state license required if applicable Valid driver's license and car insurance required COVID vaccination required; declinations will not be approved OASIS and start of care experience strongly preferred Additional Information Evaluate and treat patients to improve mobility and independent function while developing individualized care plans Coordinate and communicate with patients, families, physicians, and staff in line with hospital values Complete timely documentation of all patient care activities in the electronic medical record Understand admission processes, treatment costs, coding, and appropriate billing Ability to transfer and position patients as needed Local candidates will be considered if within the designated territory No more than 5 days of RTO can be requested during the assignment
