Job Description

Olympia Fields Hospital seeks a compassionate Physical Therapist to provide evidence-based inpatient and outpatient rehabilitation to diverse adult and geriatric patients. You will evaluate functional status, develop individualized treatment plans, deliver therapeutic exercise, manual therapy, and gait/balance training, and educate patients and families on home programs and safety. Collaborate closely with physicians, nurses, and other therapists, document accurately in the EMR, and contribute to quality, safety, and performance improvement initiatives in our patient-centered, learning-focused environment. Responsibilities Conduct comprehensive physical therapy evaluations for inpatient and outpatient populations Develop and implement individualized, evidence-based treatment plans Provide therapeutic exercise, manual therapy, and gait/balance training Educate patients and families on home exercise programs, safety, and self-management Collaborate with physicians, nurses, and rehab staff to coordinate care Document assessments, progress notes, and discharge plans in the EMRParticipate in quality improvement and patient safety initiatives Supervise or mentor students or new staff as appropriate Required Skills Physical therapy evaluation Treatment planning Therapeutic exercise Manual therapy techniques Gait and balance training Neuro and ortho rehab Patient and family education EMR documentation Interdisciplinary collaboration Outcome measurement
