Job Overview We are looking to add a dedicated and energetic Physical Therapist to join our outpatient rehab team. In this vital role, you will assess, develop, and implement personalized treatment plans to help patients regain mobility, reduce pain, and improve overall quality of life. Your expertise will support a diverse patient population across post surgicial, generalized weakness, post injury, and more for all ages. You will collaborate closely with healthcare professionals to deliver comprehensive care that aligns with clinical standards and patient needs. Responsibilities Conduct thorough patient evaluations to determine physical therapy needs, including assessments of physiology, anatomy, and medical history Develop and implement individualized treatment plans focused on patient rehabilitation goals across orthopedic, neurological, pediatric, and geriatric populations Administer various therapeutic interventions such as trigger point therapy, mobility exercises, manual therapy techniques, and patient education Document all patient interactions accurately in electronic health records (EHR) systems following clinical documentation standards Monitor progress regularly and adjust treatment plans based on patient response and evolving needs Educate patients and their families on care plans, home exercises, and community resources to promote ongoing recovery Requirements Valid license to practice as a Physical Therapist in the relevant jurisdiction Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques Experience working with geriatric patients, pediatrics, neurology cases, orthopedics, or outpatient rehabilitation centers is highly desirable Ability to develop comprehensive care plans and discharge strategies tailored to individual patient needs Proficiency in electronic health records (EHR) management and EMR systems for accurate documentation Excellent client communication skills with the ability to educate patients effectively about their treatment plans Knowledge of outpatient clinic operations and home & community care practices is advantageous
