Job Description
Signature HealthCARE seeks a compassionate Registered Nurse to provide resident-centered care in our long-term and post-acute facility. The RN will deliver skilled clinical care, administer medications, monitor conditions, coordinate with an interdisciplinary team, and support individualized care plans that promote dignity, recovery, and quality of life. Responsibilities Provide direct, resident-centered nursing care in long-term and post-acute settings Assess, plan, implement, and evaluate individualized care plans Administer medications and treatments safely and accurately Monitor resident status, document changes, and communicate with providers and families Collaborate with an interdisciplinary team (therapy, social services, physicians) Supervise and mentor CNAs and other support staff Maintain accurate, timely clinical documentation in EHR systems Ensure compliance with infection control, safety, and regulatory standards Educate residents and families on conditions, treatments, and care strategies Participate in quality improvement initiatives and team meetings Required Skills Medication administration Patient assessment Care planning Wound care management IV therapy and phlebotomy Electronic health record (EHR) documentation Infection control practices Geriatric and long-term care nursing Care coordination and discharge planning Vital signs monitoring and interpretation
