You'll work closely with physicians, nurses, therapists, and caregivers to develop and implement individualized treatment plans that improve quality of life and promote successful recovery.

Responsibilities Include:

Complete comprehensive patient evaluations and assessments Develop and implement individualized therapy plans of care Assess strength, mobility, gait, balance, range of motion, and functional abilities Provide therapeutic interventions focused on improving independence and safety Educate patients and caregivers regarding treatment plans, exercises, equipment, and home safety Communicate patient progress and condition changes to physicians and the care team Participate in interdisciplinary case conferences Assist with discharge planning and transition of care Supervise Physical Therapist Assistants in accordance with state regulations and agency policy Provide consultation and guidance to families and caregivers as needed Maintain timely and accurate clinical documentation What Makes Home Health Different?
