Develop and implement individualized treatment plans to maximize functional outcomes. Provide hands-on therapeutic interventions and supervise Physical Therapist Assistants (PTAs). Collaborate with physicians, nursing staff, and the interdisciplinary team to coordinate patient care. Educate patients, families, and caregivers on treatment plans and home exercise programs. Maintain accurate documentation in accordance with regulatory and facility standards. Mentor therapy staff while promoting best clinical practices and quality patient care. Qualifications Master's or Doctorate degree in Physical Therapy. Active, unrestricted Kentucky Physical Therapist license (or ability to obtain prior to start). Skilled nursing, rehabilitation, or long-term care experience preferred. Strong clinical assessment, communication, and leadership skills. Passion for geriatric rehabilitation and helping patients achieve their highest level of independence.
