Physical Therapist (PT)

Job Description

Job Summary NeuLife Rehabilitation of Kentucky provides specialized residential rehabilitation services for individuals recovering from traumatic brain injuries, spinal cord injuries, and other neurological conditions. Our interdisciplinary team is committed to helping each person served achieve the highest level of independence, safety, and quality of life. We are seeking a compassionate and skilled Physical Therapist to join our team. In this role, you will provide individualized therapy services focused on improving mobility, strength, safety, and functional independence while collaborating closely with the interdisciplinary care team. Responsibilities Conduct physical therapy evaluations and develop individualized treatment plans Provide skilled therapy services based on functional goals and clinical needs Monitor progress and modify treatment interventions as appropriate Promote mobility, safety, and independence in daily activities Communicate changes in condition, safety concerns, or barriers to progress Collaborate with physicians, nursing staff, case managers, and other interdisciplinary team members Complete all documentation accurately and within required timeframes Ensure documentation supports medical necessity, compliance standards, and billing requirements Maintain accurate records of all services provided and hours worked Participate in care planning and interdisciplinary communication as needed Perform other duties as assigned Qualifications Current Kentucky Physical Therapist (PT) license required Graduate of an accredited Physical Therapy program Experience in rehabilitation, neurological rehabilitation, long-term care, or healthcare settings preferred Strong assessment, treatment planning, and clinical skills Excellent communication and documentation abilities Ability to work effectively within an interdisciplinary team environment Physical Requirements Ability to assist individuals served with mobility, transfers, positioning, and physical activities Ability to stand, walk, bend, reach, and remain active throughout the workday Ability to safely perform duties in a rehabilitation environment Why Join NeuLife?
