Job Description

Cardinal Hill Rehabilitation Hospital, a leader in rehab services within Encompass Health, is seeking a dedicated Physical Therapist to join our Healthcare & Medical Services team. In this role, you will evaluate, plan, and implement individualized therapy programs focused on maximizing patient recovery and independence. Responsibilities Perform comprehensive physical therapy evaluations for patients with orthopedic, neurologic, and medically complex conditions. Develop and implement individualized, evidence-based treatment plans focused on functional recovery and independence. Provide therapeutic interventions including therapeutic exercise, manual therapy, gait and balance training, transfer and mobility training, and patient-specific modalities. Regularly reassess patients, modify plans of care, and document progress using EMR and standardized outcome measures. Collaborate with an interdisciplinary rehab team to set goals, coordinate care, and participate in patient care conferences. Educate patients and families on diagnosis, rehabilitation plan, home exercise programs, safety, and strategies to support long-term functional gains. Contribute to discharge planning and recommend appropriate equipment, services, and follow-up therapy. Maintain accurate, timely clinical documentation that meets regulatory, payer, and hospital standards. Participate in quality improvement initiatives, clinical education, and continuing professional development. Support and mentor students or less experienced clinicians as needed and uphold a culture of integrity, compassion, and teamwork.
