Cardinal Hill Rehabilitation Hospital, part of Encompass Health, seeks a dedicated RN for our night shift inpatient rehabilitation unit in Lexington, KY. In this role, you'll provide comprehensive, post-acute nursing care to patients recovering from stroke, brain and spinal cord injuries, and other complex conditions. You will perform assessments, administer medications, coordinate care with an interdisciplinary team, educate patients and families, and support safe discharge planning. Responsibilities Provide comprehensive night shift nursing care to adult rehabilitation patients Perform ongoing patient assessments and document in the EHRAdminister medications, treatments, and IV therapies safely and accurately Develop and update individualized care plans in collaboration with the team Coordinate with physicians, therapists, and case managers on patient goals Educate patients and families on conditions, treatments, and discharge plans Monitor for changes in condition and respond to emergencies promptly Follow infection control, safety, and quality standards at all times Support safe patient mobility, transfers, and use of assistive devices Participate in interdisciplinary rounds and contribute to quality improvement initiatives Required Skills Acute care nursing Rehabilitation nursing Patient assessment Medication administration Care planning and coordination Wound care management IV therapy and infusion Electronic health records (EHR) documentation Patient and family education Clinical risk and safety management
