Job Description
Community Health Network (CHN) has been placing experienced nurses and physical therapists in home care, hospice, and case management for over 30 years. We offer a stable, supportive, and growing organization that focuses on your needs and interests. Currently we are assisting one of our top home care agency partners with a need for an experienced Home Care Physical Therapist .
Coverage territory:
Burlington, Bedford, Concord, Lexington, Andover, Reading, Wakefield, MA areas. Monday-Friday, 40 hours/week Every 6th weeend rotation Highly competitive pay rate plus IRS mileage reimbursement 13 week contracts Requires 1 year of recent Home Health Physical Therapy experience doing OASIS visits with a Medicare certified agency.Great benefits include:
