This is a flexible per diem opportunity for PTs who want additional income, control over their schedule, and meaningful one-on-one patient care without taking on a full-time caseload. • Start of Care:

  $160

• Initial Evaluation:

  $120

• Re-Evaluation:

• PT Discharge:

• Routine PT Visit:

  $65 What you will do:
• Complete in-home PT evaluations, reassessments, discharges, and routine visits
• Build practical treatment plans around the patient's goals, safety, function, and home environment
• Provide skilled therapy including therapeutic exercise, gait training, balance work, transfer training, fall prevention, mobility training, and home exercise instruction
• Educate patients and caregivers so progress carries over between visits
• Document visits accurately and on time
• Communicate with nursing, physicians, and office support when care coordination is needed What you need:
• Active Massachusetts Physical Therapist license
• At least 1 year of PT experience
• CPR certification
• Reliable transportation for visits in and around Leominster
• Strong clinical judgment and comfort working independently in the field
• Professional communication and timely documentation habits
• Ability to pass a background check This is not a high-pressure, one-size-fits-all role.
