  • 52/hour Post Rehab is seeking a Physical Therapist (PT) to join our rehabilitation team.
This opportunity is ideal for a therapist who enjoys helping residents regain mobility, improve function, and achieve meaningful recovery in a skilled nursing and post-acute care setting.
Position Responsibilities:
Complete comprehensive physical therapy evaluations and develop individualized treatment plans Provide treatment focused on gait training, balance, strengthening, endurance, mobility, transfers, and pain management Reassess resident progress and modify treatment plans to maximize functional outcomes Collaborate with Physical Therapist Assistants, Occupational Therapists, Speech-Language Pathologists, nursing staff, physicians, and families Educate residents and caregivers on exercises, mobility techniques, and safety strategies Complete accurate and timely documentation in accordance with facility and regulatory guidelines

Qualifications:
