Jackson Therapy Partners is seeking a Physical Therapist to provide high-quality, evidence-based care across diverse clinical settings. Responsibilities Conduct comprehensive physical therapy evaluations, including assessment of strength, range of motion, balance, gait, posture, and functional mobility Develop and implement individualized, evidence-based plans of care to improve mobility, function, and quality of life Provide therapeutic interventions such as exercise, neuromuscular re-education, balance and gait training, manual therapy, and functional mobility training Educate patients and caregivers on home exercise programs, safety, equipment use, and strategies to prevent injury and maintain functional gains Document evaluations, daily treatment notes, progress reports, and discharge summaries accurately and in a timely manner in accordance with regulatory and facility standards Collaborate and communicate effectively with physicians, nurses, occupational and speech therapists, case managers, and other team members to coordinate care Adjust treatment plans based on ongoing assessment of patient progress, functional outcomes, and patient goals Ensure compliance with state practice acts, professional standards, payer requirements, and facility policies and procedures Contribute to a positive, respectful, and supportive work environment that reflects Jackson Therapy Partners' culture and values Participate in team meetings, case conferences, in-service training, and quality improvement initiatives as requested Provide mentorship or guidance to therapy assistants, new graduates, or students when appropriate Maintain professional competence through continuing education and staying current with best practices in physical therapy Required Skills Physical therapy evaluation and diagnosis Development of individualized plans of care Orthopedic and neurologic rehabilitation techniques Manual therapy techniques (e.g., joint mobilization, soft tissue mobilization) Therapeutic exercise prescription and progression Gait, balance, and functional mobility training Neuromuscular re-education Pain management strategies within scope of PT practice Patient and caregiver education Use of assistive devices and durable medical equipment Electronic medical record (EMR) documentation and clinical reporting Interdisciplinary communication and care coordination Time management and caseload organization Knowledge of state PT practice acts and regulatory requirements Outcome measurement and functional assessment tools
