Job Description

Hope Network is seeking a compassionate Physical Therapist to provide evidence-based rehabilitation services within our Healthcare & Medical Services team. In this role, you will assess, plan, and deliver individualized treatment to individuals with neurological, orthopedic, and complex conditions, helping them maximize independence and quality of life. You will collaborate closely with an interdisciplinary team, document outcomes, and educate patients and families. Responsibilities Evaluate patients and develop individualized treatment plans to restore function and reduce pain Provide evidence-based therapeutic exercises, manual therapy, and functional training Document assessments, progress notes, and outcomes in EMR accurately and timely Collaborate with an interdisciplinary team to coordinate holistic patient care Educate patients and caregivers on home exercise programs and injury prevention Participate in case conferences, quality improvement, and community initiatives Supervise and mentor support staff and students as appropriate Ensure compliance with regulatory, ethical, and safety standards Required Skills Orthopedic assessment Neurological rehabilitation techniques Therapeutic exercise prescription Manual therapy Gait training Patient evaluation and documentation (EMR) Treatment care planning Pain management modalities (e.g., ultrasound, e-stim) Outcome measurement and reporting Interdisciplinary team collaboration
