We are seeking a physical therapist with experience in home health care to join our team. The successful candidate will work with patients and their families to determine goals that will assist in their healing process and lead to a more independent lifestyle. NOTE:

This position is for adults and elderly adult patients only. No pediatric patients. Wayne County --> Westland, Livonia, Canton, Garden City, Dearborn, Dearborn Heights, Inkster, Wayne and surrounding areas

Responsibilities:

Provide home health care. Help patients recover from injuries and illnesses. Work under the supervision of a physician or other licensed health care provider. Complete treatment plans, evaluations, and progress notes. Engage in weekly meetings with evaluators and other health providers. Establish rapport with patients, their families, and caregivers. Provide physical therapy services and instruct patients in their proper use. Diagnose patient's conditions and prescribe treatments and therapies. Maintain accurate medical records.

Pay:

$75.00 - $120.00 per hour
