Qualifications:

Active and unrestricted Physical Therapy license Previous clinical experience in a hospital or acute care setting preferred Proficiency with EPIC or similar EMR systems highly desired Strong clinical reasoning, assessment, and patient evaluation skills Exceptional communication, documentation, and interpersonal abilities Ability to work collaboratively within a multidisciplinary team CPR certification (current)

Responsibilities:

Deliver thorough assessments and develop individualized care plans for patients Provide therapeutic interventions and evaluate patient progress towards established goals Educate patients and their families on treatment plans and techniques for continued rehabilitation Document all care activities, progress notes, and outcomes accurately in EPIC Collaborate with nurses, physicians, social workers, and other healthcare professionals to optimize patient care Assist with discharge planning and follow-up care recommendations Maintain compliance with regulatory, professional, and facility standards
