This opportunity is different. At Vitality At Home , we are opening a new brick-and-mortar Physical Therapy and wellness location in Grand Rapids, Michigan, and we are looking for an experienced Founding Physical Therapist who wants to help build it with us. You will not simply inherit a schedule. You will help establish how Physical Therapy is delivered, how patients move through the clinic, how the clinical space functions, what systems therapists use, and what exceptional patient care looks like from day one. You will begin as the primary treating Physical Therapist at the location, providing one-on-one, 60-minute Physical Therapy appointments while partnering closely with ownership to create the clinical systems and patient experience the clinic will grow around. As the location expands, this role is designed to grow with it. The long-term opportunity is to move into a Clinic Director position , helping recruit, mentor, and lead the clinicians who join the practice after you. Patients are scheduled in 60-minute, one-on-one appointments , giving you time to evaluate thoroughly, provide hands-on treatment, progress exercises intentionally, educate patients, and build meaningful relationships. You will help shape the clinic from the beginning. Your clinical experience will directly influence the treatment space, equipment, workflows, documentation systems, scheduling structure, and patient experience. Your work will go beyond helping someone recover from an injury. As the clinic grows, the Founding Physical Therapist is expected to develop toward Clinic Director responsibilities , including hiring, mentoring, staffing, scheduling capacity, clinic performance, and strategic planning with ownership. That means your clinical experience, operational insight, and ideas will help determine how this location operates for years to come. What You Will ReceivePractice Physical Therapy With Time to Actually Treat the Patient Great Physical Therapy requires more than getting someone through today's appointment. It requires understanding what is limiting them, what they want their life to look like, and what needs to change to help them get there. At Vitality At Home, you will have 60 minutes of dedicated one-on-one time with each patient. You will evaluate the whole person, establish an individualized plan of care, provide hands-on treatment, progress strength and mobility, educate patients, and guide them from their first visit through discharge. Your target caseload will be approximately 40 patient visits per week , allowing the clinic to maintain a high standard of individualized care while continuing to grow. The patients you work with may be managing orthopedic conditions, age-related strength or mobility changes, neurological conditions, balance limitations, or other challenges affecting their ability to remain active and independent. The goal is not simply to help someone hurt less. It is to help them move better, stay stronger, remain independent, and continue doing the things that make life meaningful. Help Build a Clinic From the Ground Up This is where the opportunity becomes especially unique. You will work directly with ownership to help create the environment where future Physical Therapists, PTAs, support staff, and patients will spend their time.
You will help:
Shape the clinical layout and patient flow Plan treatment, gym, equipment, front desk, and waiting areas Recommend clinical equipment, modalities, and supplies Create evaluation and treatment workflows Develop documentation templates and discharge criteria Help configure the EMR and scheduling systems Establish appointment types, durations, and scheduling templates Help develop cancellation and scheduling policies Design the patient experience from the first phone call through discharge follow-up Assist with clinic-opening readiness and Michigan compliance requirements Help establish soft-launch scheduling and opening procedures You will not be expected to figure everything out alone. You will work closely with ownership, but we want a Physical Therapist who enjoys thinking beyond the treatment table and helping build better systems. Your Role as the Founding Physical Therapist Your primary responsibility will always begin with excellent patient care.
You will:
Perform comprehensive Physical Therapy evaluations Develop individualized plans of care Provide one-on-one Physical Therapy treatment Use hands-on treatment, therapeutic exercise, functional training, neuromuscular rehabilitation, and other appropriate interventions Progress patients toward meaningful functional goals Educate patients and families throughout the rehabilitation process Complete timely, accurate clinical documentation Maintain compliance with payer requirements and the Michigan Physical Therapy practice act Create an exceptional patient experience from evaluation through discharge Help establish the clinical quality standard future therapists will follow Build the Systems Behind Great Patient Care As patient volume grows, you will also help evaluate what is working and improve what is not. You will help monitor and improve: Patient visit volume Cancellation and no-show rates Documentation timeliness Patient outcomes Scheduling capacity Clinical workflows Patient experience Operational efficiency You will also have opportunities to help build relationships with local physicians, specialists, healthcare providers, and community organizations throughout Grand Rapids. Who We Are Looking For This is an experienced clinician role. You may be a great fit if you: Hold a Doctor of Physical Therapy degree Hold or are eligible for a Michigan Physical Therapist license Maintain current CPR certification Have approximately 5 years of Physical Therapy experience Have experience in geriatrics, orthopedics, or neuromuscular rehabilitation Are comfortable practicing independently and making clinical decisions Deliver excellent one-on-one patient care Communicate confidently with patients, families, clinicians, and community partners Enjoy improving systems and solving problems Are comfortable providing feedback and making recommendations Have an interest in mentoring and developing other clinicians Want to grow into a leadership position Are excited by the opportunity to help build a clinic rather than simply work in one Because of the level of clinical independence and leadership involved, this position is not designed for a new graduate. Mentorship and collaboration will still be available. We simply need someone who already has a strong clinical foundation and is ready to take greater ownership.
