Renewal Rehab is seeking a compassionate Physical Therapist Assistant to join our on-site rehab team serving seniors in skilled nursing and post-acute care settings. As a PTA, you will implement individualized treatment plans under the direction of a Physical Therapist, focusing on improving strength, mobility, balance, and functional independence. You'll document progress in the EMR, collaborate closely with nursing and rehab staff, and educate patients and families. Responsibilities Provide physical therapy treatments under the direction of a licensed Physical Therapist Implement individualized care plans to improve strength, mobility, balance, and function Accurately document treatments, patient progress, and outcomes in EMR systems Assist with patient assessments and communicate changes in condition to the PT and care team Educate patients and families on exercises, safety, and home recommendations Collaborate with nursing, rehabilitation, and interdisciplinary teams to support care plans Adhere to Medicare, regulatory, and facility documentation and billing standards Participate in team meetings, case conferences, and quality improvement initiatives Support a patient-centered, compassionate, and ethical care environment Engage in continuing education and mentorship opportunities to enhance clinical skills Required Skills Physical therapy interventions Therapeutic exercise prescription Gait and balance training Patient assessment and documentation EMR proficiency Care planning with interdisciplinary teams Geriatric rehabilitation Fall prevention strategies Patient and family education Time management
