Physical Therapy License Full Job Description Overview:
The Physical Therapist is responsible and accountable for residents' evaluations, ongoing assessments, and the planning and implementation of therapy interventions.Responsibilities:
Develops and revises plan of care based on the residents' needs and establishes functional goals, outcomes, and discharge plans. Plans for residents' discharge and anticipates for long term and/or short term residents' needs. Educates other health care team members, residents, family, and caregivers on information regarding the care plan for the residents.Qualifications:
