Job Description
Astera Health is looking for a full-time Physical Therapist to join our Rehabilitation Services team. This position is scheduled for 80 hours per bi-weekly pay period, Monday-Friday in Henning with inpatient weekend rotation every 7-8 weeks in Wadena. Responsibilities required, but not limited to: Ability to operate independently at the Henning PT clinic, while maintaining strong communication and collaborative relationships with the broader Astera team to ensure consistency of care and culture Formulates total treatment programs, adjusts program to each patient's needs and condition to ensure a positive outcome. Carries out treatments in accordance with physician's orders. Contacts physicians as needed concerning patient condition or progress to maintain continuity of care. Documents evaluation and progress notes for each patient treated. Follows established policies, procedures, and safety precautions with use of modalities and equipment to prevent injury. Provides patient care in a safe manner during treatments. Provides physical therapy services as requested by Astera Health to meet outreach needs. Complete all documentation in adherence to Astera Health and CMS standard with 24 hours of treatment session Will work with in the rural general setting for treatment in areas of orthopedics, athletes, and neurological therapy across all ages.
