The environment is right in front of you. You'll evaluate strength, balance, gait, transfers, endurance, mobility, fall risk, and functional limitations while seeing exactly what the patient needs to navigate every day. Then you can build a plan around goals that actually matter to them. Getting upstairs. Walking safely to the mailbox. Transferring into their own shower. Getting out of bed without assistance. Moving through the house without another fall. What You'll Do Perform comprehensive in-home Physical Therapy evaluations Develop individualized plans of care based on functional goals Provide skilled interventions addressing strength, balance, gait, mobility, transfers, and endurance Evaluate fall risks and home safety concerns Recommend appropriate equipment and modifications Educate patients and caregivers on safe mobility Monitor progress and modify treatment plans when appropriate Support patients recovering from hospitalization, surgery, illness, or functional decline Collaborate with physicians, nurses, PTAs, OTs, and the interdisciplinary team Complete accurate and timely clinical documentation Travel throughout Southwest Minnesota based on patient needs Bring Your PT Experience Into the Home We're interested in Physical Therapists from Home Health, Outpatient Orthopedics, Acute Care, Inpatient Rehabilitation, Skilled Nursing, Transitional Care, Senior Living, Long-Term Care, Hospital Rehab, and other clinical therapy settings . Required Qualifications Active Minnesota Physical Therapist (PT) license Valid driver's license and reliable transportation Willingness to travel throughout Southwest Minnesota Strong clinical assessment and decision-making skills Excellent patient and caregiver communication Ability to independently manage a field-based schedule Preferred Qualifications Home health experience OASIS experience Geriatric, post-acute, or medically complex patient experience See What Your Patient Is Actually Trying to Get Back To Physical Therapy isn't ultimately about performing well in a treatment session.
