Duties Assess patients' physical conditions by evaluating mobility, strength, and functional capabilities using medical terminology and anatomy knowledge. Develop and implement tailored care plans that incorporate physiotherapy techniques such as trigger point therapy and orthopedics interventions to meet individual recovery goals. Conduct discharge planning to ensure seamless transition from hospital or clinic to home or community settings, coordinating with multidisciplinary teams for optimal patient outcomes. Administer treatments in various environments including inpatient units, outpatient clinics, home health visits, and pediatric or geriatric care settings. Utilize EMR (Electronic Medical Records) systems to document patient progress accurately and maintain comprehensive medical records. Educate patients and their families on physiotherapy exercises, injury prevention strategies, and the importance of ongoing care for sustained health benefits. Collaborate with healthcare professionals to refine treatment approaches based on patient response and evolving medical conditions. Experience Proven experience in acute care hospital settings with a strong understanding of inpatient procedures and protocols. Hands-on experience in outpatient clinics or physiotherapy practices specializing in orthopedics, geriatrics, pediatrics, or home & community care. Knowledge of physiology, anatomy, and medical terminology essential for accurate assessment and treatment planning. Familiarity with EMR systems used in healthcare environments to ensure efficient documentation and communication. Demonstrated ability to develop effective care plans that incorporate discharge planning and patient education strategies. Prior work experience in home health or community-based programs preferred to support diverse patient needs outside traditional clinical settings.
