Position Overview:
We are seeking a licensed and motivated Physical Therapist (PT) to evaluate, plan, and deliver rehabilitative services to residents recovering from injury, illness, or surgery. This position plays a vital role in restoring function, improving mobility, and enhancing the quality of life for those we serve. Key Responsibilities:
Perform patient evaluations and develop individualized treatment plans based on physician orders. Deliver physical therapy treatments and adjust plans as needed to achieve functional goals. Supervise PT Assistants, rehab techs, and therapy students as required. Maintain accurate and timely documentation in accordance with regulatory standards. Collaborate with interdisciplinary team members, including nurses, physicians, and occupational therapists. Educate residents, families, and staff on therapeutic techniques and home exercise programs. Participate in discharge planning and facility quality improvement initiatives. Travel or float to nearby facilities, as needed.
Qualifications:
Graduate of an accredited Physical Therapy program. Current state licensure or eligibility to obtain one. Experience in a skilled nursing or long-term care setting is preferred but not required.
