Job Description
Liberty Healthcare seeks a dedicated Home Care Physical Therapist (PT) to join our team, providing in-home treatment and rehabilitation services tailored to individual patient needs. This role is critical in helping patients achieve their highest level of mobility and independence within their home environments. As a Liberty Healthcare PT, you will work with diverse patient populations, utilizing innovative techniques and treatments backed by our commitment to quality care and personalized recovery plans. Responsibilities Conduct comprehensive patient assessments to establish a personalized care plan. Implement treatment plans that incorporate therapeutic exercises, manual therapy techniques, and other modalities as needed. Evaluate and document patient progress, modifying treatment plans as necessary. Educate patients and their families on post-treatment care and wellness. Maintain accurate and confidential patient records. Collaborate with other healthcare professionals to ensure comprehensive care. Comply with all state and federal regulations regarding physical therapy. Required Skills Strong knowledge of musculoskeletal, cardiovascular, and neurological disorders. Excellent communication and interpersonal skills. Ability to work independently in a home-care setting. Strong organizational and time-management skills. Proficiency in using physical therapy equipment and tools.
