Position Overview:

A Physical Therapist (PT) is responsible for delivering home health care services to help patients recover from illness or injury in their own homes. The role involves performing assessments, developing and modifying plans of care, and ensuring documentation compliance. The PT collaborates with healthcare teams, educates patients and caregivers, supervises staff (PTA, HHA), and completes OASIS assessments where permitted.

Key Responsibilities:

Conduct initial and ongoing evaluations of mobility, strength, and function Develop, implement, and modify individualized treatment plans Document treatment progress and discharge plans effectively Coordinate care with healthcare team members and educate patients/caregivers Supervise clinical personnel and complete assessments per regulations

Qualifications & Skills:

Degree in Physical Therapy from an accredited program Unrestricted state license as a Physical Therapist Proficiency in assessments, documentation, and compliance

Preferred:

strong communication, organization, attention to detail, and experience in home health and OASIS documentation
