Work Environment This role is primarily onsite, providing physical therapy treatment in patients' homes. Key Responsibilities Conduct thorough assessments of patients with functional limitations and disabilities, developing individualized treatment plans based on their unique needs. Provide consultation to other healthcare providers regarding patients' continuity of care, ensuring accurate information exchange for optimal outcomes. Deliver compassionate care while adhering to customer service standards, ensuring respectful engagement with patients of all ages and backgrounds. Evaluate treatment effectiveness and make necessary adjustments, documenting progress and submitting billing information promptly according to organizational policies. Maintain effective verbal and written communication with interdisciplinary team members, physicians, and referral sources to facilitate a cohesive treatment approach. Participate actively in leadership functions including community outreach programs, continuous quality improvement initiatives, and marketing efforts to promote our services. Engage in professional development opportunities such as attending staff meetings, in-services, and Continuing Education programs, ensuring compliance with licensure requirements. Patient Population / Client Focus The successful candidate will serve a diverse patient population, focusing primarily on individuals requiring home-based physical therapy services. This includes elderly patients recovering from surgery or illness, individuals with chronic pain, and those needing rehabilitation due to injury or other medical conditions. By delivering tailored treatments in the patients' living environments, our therapists play a key role in promoting recovery and enhancing the patients' capacity to live independently. Qualifications A Bachelor's Degree in Physical Therapy from an accredited institution is required. Current state licensure in Physical Therapy and certification in Basic Life Support (BLS) are mandatory. Strong written and verbal communication abilities to effectively convey information to patients and collaborate with the healthcare team. Proficiency in word processing and familiarity with electronic medical record systems to document treatments accurately. Preferred Experience Candidates should possess a minimum of two years of experience in a clinical setting, ideally within home health or outpatient care. Experience in managing diverse patient populations and knowledge of home care protocols will be advantageous. Compensation and Benefits Atmore Physical Therapy offers a competitive annual salary ranging from
USD 100,000
to USD 120,000, commensurate with experience.
