Tender Touch Rehab Services is seeking a compassionate Physical Therapist Assistant to deliver high-quality, patient-focused care in skilled nursing and long-term care settings. The PTA will implement individualized treatment plans under the direction of a Physical Therapist, focusing on improving mobility, strength, balance, and safety for geriatric and sub-acute residents. Responsibilities Implement PT treatment plans for geriatric and sub-acute residents under supervision of a Physical Therapist. Provide therapeutic exercises, gait and balance training, and functional mobility interventions. Assist patients with transfers, bed mobility, and use of assistive devices to improve safety and independence. Accurately document treatments, patient progress, and outcomes in EMR systems in a timely manner. Collaborate with an interdisciplinary team, including nursing and other rehab disciplines, to coordinate care. Educate patients, families, and caregivers on exercises, safety strategies, and use of equipment. Monitor patient response to treatment and report changes or concerns to the supervising Physical Therapist. Adhere to federal, state, and facility regulations, ethical standards, and company policies. Maintain a safe, clean treatment environment and follow infection control and safety protocols. Participate in professional development and in-service training to support clinical growth and quality care.
