MULTIPLE LOCATIONS IN MONMOUTH COUNTY & MIDDLESEX COUNTY
•We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team. The ideal candidate will possess a strong understanding of anatomy and physiology, along with a passion for providing exceptional patient care. In this role, you will develop individualized care plans, assist patients in their rehabilitation journey, and ensure effective discharge planning. Your expertise in various therapeutic techniques will be essential in helping patients achieve their mobility and functional goals.
Responsibilities:
Conduct comprehensive assessments of patients to determine their physical therapy needs. Develop and implement personalized care plans tailored to each patient's specific conditions and goals. Provide hands-on treatment using various therapeutic techniques, including trigger point therapy. Educate patients and their families on exercises, home care strategies, and lifestyle modifications to enhance recovery. Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes. Collaborate with other healthcare professionals to coordinate patient care across different settings such as hospitals, home health, and community care. Maintain accurate documentation of patient evaluations, treatments, and progress notes in compliance with regulatory standards. Participate in discharge planning to ensure a smooth transition for patients returning home or to other care facilities.
Job Requirements:
A degree in Physical Therapy from an accredited program is required. State of NJ Physical Therapy License is required in good standing. Proven experience in patient care within settings such as outpatient orthopedics or sports rehabilitation is preferred. Strong knowledge of anatomy and physiology is essential for effective treatment planning. Familiarity with developing care plans and conducting discharge planning processes is highly desirable. Excellent communication skills to effectively interact with patients and multi-disciplinary teams. Ability to work collaboratively within a team while also demonstrating independent decision-making skills.
