Work Environment This is a full-time position, requiring 40 hours a week, with some variability on weekends based on patient needs. As a physical therapist in a home care setting, you will have the unique opportunity to work closely with patients in their own environments. This approach not only fosters a more personal connection but also allows for tailored interventions that can lead to improved recovery outcomes. Key Responsibilities Conduct thorough examinations and evaluations of patients to develop effective treatment plans. Consult with other healthcare providers to ensure a comprehensive continuum of care for patients. Interact professionally and empathetically with patients of all ages and cultural backgrounds, adhering to high customer service standards. Implement treatment plans, re-evaluate patient progress, and educate families and caregivers. Maintain detailed documentation of patient evaluations, treatments, and outcomes, while complying with all applicable laws and professional standards. Engage in interdisciplinary collaboration, maintaining open communication with other healthcare professionals, and participating in team meetings. Participate in community outreach and educational initiatives to promote health and wellness. Guide and supervise any assigned support staff, ensuring the efficient use of department resources. Stay informed about the latest research and best practices in physical therapy through ongoing education and training. Patient Population / Client Focus Your patient focus will primarily be on individuals needing rehabilitation due to orthopedic conditions, sports injuries, post-operative care, and chronic pain management. Your role will be crucial in helping patients regain their mobility and independence through tailored, evidence-based interventions. Qualifications Bachelor's Degree in Physical Therapy required; Master's or Doctorate preferred. Current state license in Physical Therapy, ensuring compliance with all regulatory requirements. Strong verbal and written communication skills to effectively convey treatment plans and progress to patients and team members. Proficiency in using electronic health record (EHR) systems and other relevant software applications. Preferred Experience Previous experience in a home care or community-based therapy setting. Demonstrated ability to work collaboratively as part of an interdisciplinary team. Familiarity with evidence-based practice and outcome measures in physical therapy. Compensation and Benefits This position offers a competitive annual salary ranging from
USD 100,000
to 120,000, commensurate with experience and qualifications.
