Overview We are seeking a Full-Time Physical Therapist (PT) for a very flexible home health position. This role offers meaningful one-on-one patient care, manageable territories, and excellent compensation—without on-call requirements.

Key Responsibilities:

Provide physician-ordered physical therapy services in patients' homes Evaluate patient function and develop individualized plans of care Complete initial evaluations, reassessments, and plan-of-care updates every 30 days Supervise Physical Therapy Assistants and Home Health Aides as appropriate Educate patients and caregivers on exercises and home care routines Collaborate with physicians and the interdisciplinary care team Complete timely and accurate clinical documentation, including OASIS Participate in quality initiatives, in-services, and care conferences
