  • Home Health | Buffalo, NY | Full-Time, Part-Time & PRN | 3 Openings Stern at Home Therapy is expanding throughout the Buffalo, NY area and is hiring three Physical Therapists (PTs) to support our continued growth across the region.
We have opportunities spanning a broad territory from North Buffalo through the city and down into the Southtowns , allowing clinicians to build a caseload closer to where they live rather than covering the entire region. The Southtowns generally include communities such as West Seneca, Orchard Park, Hamburg, East Aurora, and surrounding areas. We are hiring for Full-Time, Part-Time, and PRN , making this a strong opportunity whether you're looking to build a primary caseload or add flexible visits around an existing position.
What You'll Do:
As a Physical Therapist with Stern at Home Therapy, you'll evaluate and treat a primarily geriatric patient population in their homes. You'll have the clinical autonomy to develop individualized plans of care while our administrative team supports you with referrals, scheduling, authorizations, and patient coordination. With three openings available , we're looking to build coverage across different sections of the Buffalo market so clinicians can develop geographically concentrated caseloads.
Responsibilities include:
Perform physical therapy evaluations and develop individualized plans of care. Provide one-on-one physical therapy treatments in patients' homes. Help patients improve strength, balance, mobility, endurance, and functional independence. Provide gait training, transfer training, fall-prevention strategies, and home exercise programs. Recommend assistive devices, durable medical equipment, and home safety modifications when appropriate. Complete accurate and timely clinical documentation. Communicate with physicians, caregivers, families, and the interdisciplinary care team. Coordinate with our administrative team to develop a caseload within your preferred Buffalo-area territory.
Compensation:
Full-Time

Salary Option:
$82,000
  • 100,000 annually.
Salary may increase based on patient volume. W-2
Tiered Per-Visit Option:
$45
  • 60 per follow-up visit.
$65 per 4-unit evaluation. Your per-visit rate increases as your monthly patient volume grows. 1099
Tiered Per-Visit Option:
$60
  • 70 per follow-up visit. $75 per evaluation. Your per-visit rate increases as your monthly patient volume grows. Part-Time

Patients:
W-2: $55 per follow-up visit. $65 per 4-unit evaluation. 1099: $62 per follow-up visit. $75 per evaluation. PRN
  • Minimum 5
Patients:
$62 per follow-up visit. $75 per evaluation. Part-time and PRN compensation is paid at a fixed per-visit rate and does not use the tiered volume model. Full-Time W-2

Qualifications:
Active Physical Therapist license in New York required. Graduate of an accredited Physical Therapy program. Reliable transportation required. Strong clinical, communication, documentation, and time-management skills. Ability to work independently in a home-based setting. Home health or geriatric experience is preferred but not required.
